In table 6.1 (in published version: table 1, page 504), there is a typo in the last entry of the third column. The entry should read 8.13.2.1 rather than 8.13.38.13.2.1.

Near the end of page 32 (in published version: page 508, near the top of the second column), both appearances of 3⁵ should read 3⁶.
